At JC Insight Therapy, your rights as a client are important. This page outlines how you can access your records, communicate concerns, and file a complaint if needed.
1. Requesting Your Health Records
You have the right to request access to your mental health records, consistent with applicable state and federal laws. I am committed to supporting your access while protecting your privacy and confidentiality.
All record requests must be submitted in writing through the secure client portal. Please refer to the Informed Consent for detailed information regarding request procedures, timelines, and applicable fees.
In some situations, a treatment summary may be provided instead of full records when clinically appropriate. This determination is made using professional judgment to support client well-being and safety.
2. Questions, Concerns, or Feedback About Care
If at any time you have questions or concerns about your therapy services, you are encouraged to discuss them directly with me. Open communication is an important part of ethical and effective care.
